Display different portal form based on selecting Country (or any customfield) from the JSD

Hi,
Is it possible to display different portal form based on selecting Country (or any customfield) from the JSD.
Customers should login to customer portal, first screen should allow to select country and job description, based on selection next form should appear to capture ticket details and saving should create JSD ticket.
Kindly let me know if any one have tried this out or could be achieved with the plugin
