I'd like to differentiate bugs that are created via Proforma from other sources (e.g. Jira Ui, API).
I'd hope to link one of the Proforma answers to the "components" or "labels", however, it seems then the dropdown options are then bound to all available components/labels (which is a longer list than those users should see).
I could create a Jira Project Automation rule, however, I don't see a field that would indicate when a bug is created via Form.
Does anyone have a suggestion ?

You could use Proforma automation that will trigger a status change. Then with the status change, you can trigger an update to a customfield that will add a label to the ticket.
Step 1: Create a transition that calls itself and name it "XXX"
Step 2: Proforma Automation

Step 3: Jira Automation
- Trigger: Issue transitioned to xxx status
- condition: issuetype = bug
- Action: add labels
Step 4: Condition on the transition XXX
- Make sure that this is hidden from the users so that only automation can call it.
