You have made a field called "Missed SLA Comments" mandatory for the issues, but not put it on the creation screen.

Either

Add it to the screen
Change the project's field configuration to make it optional
Change the workflow, removing or amending the validator on "create" to not check that field is filled

You must be a registered user to add a comment. If you've already registered, sign in. Otherwise, register and sign in.
