I am sorry, but this does not make sense.

The use of "template" suggests you want to pre-fill fields, but that usually means you've started asking users for fields that you don't really need them to fill in. 

That is a bit of a guess, but I really do not know what you mean by "to associate issue type" after the first part of the question
