Is there a way to make a dynamic interaction with a JSD portal

I have this form below, what i want is if user select yes for any of those fields then make the others fields hide or blocked, the purpose is to force the users to answer yes only on one question

can i achieve this with a script in the cration transition ? any other idea please or example on js please if it's required

Using the applications native functionality, no, however I found the following add-on app that can accomplish this:

• Dynamic Forms for Jira

The app is provided by the vendor Deviniti, and there is a really good write up in the marketplace apps section of community of the capabilities viewable here:

Alternatively, I was not able to locate any specific examples but this does seem like something that could be accomplished using either ScriptRunner for Jira or Power Scripts™ - Jira script automation and would be good alternatives to look into for a possible solution.
