Form add-on that can be embedded on website

I have been working with Smart Forms and Raley intake. While both forms have what I am looking for in terms of linking data to custom fields and being embedded without needing to open my HelpDesk to the public, neither seems to be able to create columns, so the entire form is linear, leaving it very long.

Pro Forma is not something my organization can pay for so please anything but that or ANY of the "app" apps that want me to load embeds etc (ie Jot Forms).

I would love some hints on this because I have a deadline looming and need to settle on a product.

How have you been solving this requirement?

If you haven't already, consider revisiting ProForma since it's now free, which directly addresses your previous budget concerns. Because of your needs for multi-column layouts and embedded forms linked to custom fields, this change makes a big difference.
