Requirement:
I wish to create the ticket 'Request a new account' in Jira.
Issue:
I receive the following error message after filling in all necessary information correctly: "Your request could not be created. Please check the fields have been correctly filled in."
In the 'Description', I typed the name of the person that needs the account and the e-mail address. After clicking on 'Create', I receive the error message.
Could you please look into this? Thank you in advance!

This issue happens when there is a hidden field on the request type and no one added a default value for that.
Please, go to the project settings > request types and click on "Edit fields" for "Request a new account" and check if there is any hidden field.
Another thing that may cause this is a field set as required, but for the entire project. You can check this on project settings > fields > and check if some field is required or optional. To confirm if a field is required, it will show a label on the side of the field name.

I am working for the same company as Steven (who asked the question).
I looked into your suggestions but it seems like all the setting are correct.
Although, we are not able to create a ticket as an "access request".
Do you have any other solutions?

Another configuration that may cause this issue as well is the order of the post functions or some conditions on the workflow.
Please, check the workflow for the issue type related to the "Access request" in order to confirm if there is no condition where some field needs to be filled and also compare the order of the post function of this workflow with the post functions from a workflow that it's working normally.
