Can you create an automation that on a scheduled date, creates a request and attaches a form to the

I have tried to do this, but it needs a JQL query to run for the trigger for the action of add a form to be accepted. What would be the way to work around on this?

I have a few suggestions for you.

To automatically create an issue with a form, try out Business Process Manager. Add-on allows to set up a flexible scheduler. You can turn the number of occurrences of the issue that include a form. 

And the second solution Is Smart Jira Forms plug-in.  But in this case, you need to attach the form in the Jira issue manually.
