No, it is not possible at this time to add a logo to a form in out of the box Jira. You might be able to do it with one of the third party apps for forms in the Atlassian Marketplace.
You may try Smart Forms for Jira app, developed by my team, where you can design your cusmtom form with logo, fonts, colors and more. You may add logo as embeded element from your website or drive, additionaly you can place there anything else like video or image.
