What we are looking for is when someone chooses CCB (Change Control Board) issue type that the description fields for that Issue Type auto populates with a set of questions we need answered about the issue they are about to log. So when they choose CCB in the Create Issue screen the description field will auto populate questions like...
1. What is the impact to the customer?
2. What is the target release date?
I'm sorry to say that this wouldn't be possible in the OnDemand environment because it would require significant customisation and/or a custom plugin to perform this kind of fucntion, but unfortunately plugins and customisations are not permitted on the OnDemand platform so you are reliant on the core functionality of JIRA and this isn't currently provided. Therefore I would like to invite you to read our documentation on the Restricted Functions of the OnDemand Platform to get an idea of the possibilities and limitations of the platform.
