Import JIRA issues from CSV no longer works

This question is in reference to Atlassian Documentation: Troubleshooting JIRA Problems in JIRA Agile

I am trying to import Story issues from CSV and the import process just seems to not work once you click Validate or Begin Import buttons. Did that break in one of the latest versions? A blank browser page is presented and nothing is spinning or acting like it is even doing anything. We are using JIRA 7.1.7 and attempting to use the standard Import Issues from CSV function; not using any plug-in.

And now, I see that Issue Type is no longer a JIRA field that can be mapped. The only way to use the importer now is to import issues into the default type for the project which is NEVER the issue type I need to import. Atlassian really broke this feature now to the point of it being unuseable.

I'm also experiencing this issue in version 7.1.7.  The jiraservicedesk logs indicate the following error:

Also occurs in 7.1.9.
