How do I set up my issue template for a new project?

I have a new project, and want to set up two options for tickets ~ Data Request and Tagging Request. How do I set up my page/form/issue process (not sure what to call it)?

I am pretty sure all the sections I require are there, just don't know the ABCs of how to simply set up my process. Some of the information I need is:

1. Requestor name, email, phone

2. Data request or tag request

3. Report requirements: weekly, monthly, frequency, etc.

4. Due date

5. Comments...

and so on

unfortunately, this tool is not very intuitive for me? :-(

From my understanding of the problem, I have listed down the documentations to assist you on your project creation.

  1. You mentioned that you would like to setup 2 options for tickets, that can be done actually by creating an issue type to associate with it. Refer to the link for details

  2. As for the Request name, email, phones and etc, you might want to create custom fields to host all that information. You can do so by referring to the link below

  3. I would suggest you to have a look at how to configure a customized workflow based on your requirement and associate different screens (or page/form) on the transitions to capture more information from your client
