We are in the process of configuring a service desk.
Depending on the request type we have a complete different set of questions to be answered.  Using custom fields doesn't cut it, given the complexity of the forms.

 What would be a good approach to implement such forms ?

You can setup the request type at:

  • Project Settings
  • Click on Request Type
  • Click on Edit Fields
  • Click on Add a field

So you can setup as you wish.

Yes - that is correct, but this requires for each question a separate custom field.  We want to avoid to create > 500 custom fields to gather answers on different types of questions
What we are looking for is the capability to link a form containing  all these questions (and answers).

For instance when I want to submit an expense note, I don't want to have tens of customfield to reflect every line item

Francis, you could try Jirassimo IntakeForms It lets you show and hide fields dynamically depending on answers chosen.
