How to personalize request form per customer?

Hello!

On one of my JSM projects, external customers are able to raise incidents from the Customer Portal.
However, writing customer name, ID, and location (which is mandatory) is a free text field which causes discrepancies in reporting and queueing.

I do have customer grouped in my projects.
Is there a way to set up the request forms so that if a customer from a specific group logins, there are limited options or a field is automatically filled?

Meaning:
On a form I have "Customer", "Line" and "Location" as a free text.
What I want is that if the username "jack@company.com" reports an incident, on the form it is auto-filled as Company on the "Customer" field and then on Line and Location a dropdown shows only the pre-set options for Company.com

But if "william@corporate.com" logins and reports something, then "Corporate" will show as the "Customer" field and their own pre-set "Line" and "Location" shows up only, without the ones from "Company"
