How to get Application names of JIRA instances in Confluence page?

We have created a form in Confluence and we are wishing to fetch all the application names from JIRA instance in this Confluence instance .
Next, we would like to then select application name from the drop down list that we will be having all the application names from JIRA instance. And then to show that selected data in the data sheet created.
Could you kindly suggest us of a way to get this requirement done.
