How to associate issues to user stories?

I would like to, assign issues to the stories (ie, assigning bugs/change requests to stories to group the issues for a specific user story.

So that, I can view what are all the issues reported against that specific story, to get a better view and report.

I'm not entirely sure what you are looking for here. Base on the description:

a user story is one or more sentences in the everyday or business language of the end user or user of a system that captures what a user does or needs to do as part of his or her job function

It is not feasible to breakdown a Story into several issue types. Perhaps you can elaborate further on this?
