Form for UAT feedback

I want to create a form and make it accessible to people within the company to provide feedback when they are UAT new functionality.

  1. The people who would be using the form don't have a JIRA login so how can I make it accessible to them?
  2. I want the submitted response to create a bug in the backlog of my project

Since the users providing UAT feedback don’t have Jira access, Jira’s native forms won’t work for your case. However, you can use Smart Forms for Jira to collect feedback from non-Jira users while ensuring that each response creates a bug in your backlog automatically.

Hello and welcome,

You would need to update permissions to allow anonymous creation of issues. 
Here is KB article that walks you through the process.

The Jira form you create should be shareable as a link so you can share that link. 

Please note that this method will not allow you to communicate with the reporter via the Jira issue. 

If you are planning to use Jira Service Management (JSM), this would allow  you to create a form or a widget to collect feedback. In this case, the people reporting the bug would be customers of the project. And with the right settings, anyone can be customer. This would allow you to interact with reporter via JSM.
