Email Approvals in Workflow

I'm trying to figure out a way to request approvals from a group of users (can be an "or" approval) during the "Out of Cycle Approval" status.  I would like to accomplish this without a plugin.  I did some searching and it mentioned there should be a "Include Approval Step" field but I'm not seeing it on my screen.  This a workflow in a company managed project.  Am I missing something?

Is the workflow associated with the project? You can check this in Workflow schemes.

Another thing to check is that the `Approvers' field is added through project settings > request types

@Stuart Capel - London  Yes it is associated with a project and verified it's active.  I'm not sure I understand your second point.  Can you please add a screenshot of what you're talking about?
