Difficulties with adding a custom field into a create issue form

Hi! For one specific issue type, I'd like to add a template in the description field. I wasn't able to find a solution on how to do it, so I created a custom text field with the template. Now I'd like to add it to the create issue form, however, despite it's added in the screen scheme as a create issue operation, it doesn't show up in actual create issue form.

Questions:

1. You created a custom field
2. Field is added to the create screen
3. Screen is in a Screen Scheme
4. Screen Scheme is in a Issue Type Screen scheme
5. Issue Type Screen scheme is associated with the right project

If this is all true is should work, if the project you are using is a company managed project.
