Confluence content in Jira based on field value

We create various content in confluence for particular field values in JIRA. we wanted either the content from confluence to display in JIRA or at least a pointer to the confluence page. and it has to be dynamic such that the links/content change based on the value in the JIRA field.

Not really sure how to tackle the problem, hence no screenshots. Looking for any suggestions.
