How to customize satisfaction survey?

Is it possible to add in an automatic notification on the portal?

You could use Automation so that [Proforma] "Forms" are attached to the ticket, including on the portal, so that the Reporter/Customer can fill out a "survey" form with whatever fields you want on it.

Hey @Brenno Maia 

From the tags I could see it is Jira work Management (JWM), if that is the case.

Out of the box Customer Satisfaction is not there, its only available for Jira Service Management(JSM).

If you are having JSM please follow pramodh's answer.

Even though if you have JWM you can try adding on exploring here https://marketplace.atlassian.com/search?query=survey
