unchecking checklist when issue is transitioned

i have a project set up for preventative maintenance at work and a set checklist to follow. how do i use automation to get the check list to reset when the issue is transitioned from DONE to TO DO?

1. add custom field that is of type URL

2. you can create a "create screen" for the project in question and add the new custom field to it. If you already have a create screen OR you want the field to simply be on the default screen then simply add the new field to the screen.

1. If you created a new "create screen" then you need to associate that screen to the create action.
