Are Jira Forms used primarily as a ticketing system? I created a stakeholder map, and I'm looking for a format to send a stakeholder survey. I was wondering if I can use Forms for that, but it looks like I can't put too many fields in the file.

Really depends on the product that you are using on how they can best meet your needs. "Forms allow you to collect information and capture work from other teams or stakeholders in your organization. Create different types of forms to receive different types of work from your stakeholders." is from the information on how forms can be used in Jira Work Management.
So if the project supports forms like JWM then you could use it to collect information from stakeholders.
