Creating an issue from forms

Hi,

I'm trying the new Forms feature in Jira cloud. My project is a company-managed software development project.

I have created a form template and form settings are below;

Eventhough I have admin role in Jira and in the project, when I'm trying to create issue by following the link, I get this error;

My second question is,

Is there any way to add a form to an issue when a custom field value is changed( it could be done with using Jira automation or JMWE)

Could anybody help me?
