If you go to "create issue collector" in the JIRA project, it will generate the javascript for you. There's no point us giving you the html, you're still going to need to generate it in JIRA to get anything useful out

If you're stuck on where to place it, then it's:

&lt;head&gt;
&lt;script you got from the issue collector creation stuff in JIRA&gt;
&lt;/head&gt;
