What determines whether a field is preselected (tickbox) for change on Screen 3 of 4 in the Bulk edit operation ?
We have a few 'Database Customfields' a long way down on the screen which are mysteriously pre-selected (with blank vallues). This means that super users are often caught by this and inadvertently blank out existing values.

As Fields need to be added to Screen, how do I add Attachment field to a new screen??

Ah.. you need to add the attachment field to the request type via project settings. If you can't find it, you need to first add the field to the create screen associated with the request type's issue type (more details).
Note that this adds the attachment field to the portal request creation. If the customer wants to attach a picture after request is created he/she can do that via the comment box.
