It represents how long the issue has been on the board.

I believe it's controlled under the board settings.

When you have four red dots in means it's been on the board for a long time. What "a long time" is exactly I'm not sure of.

I'm not entirely sure to be honest but I would imagine it's relatively to the sprint time frame.
