Each time user need to enter captcha along with valid credentials.

User does not have any failed count

User does not have any RSS feeds using old password however user has recently changed the password.

Clearing cookies, password from browser did not help.

Please advice.

This happens when user log ins every morning, once logged in it does not ask for captcha throughout the day.

I had this problem and it was due to a number of failed login attempts, in my case the Eclipse plugin was trying to login multiple times with an old password, the administrator (me in this case) cleared the failed user login attempts, I had to login as the user with a captcha once more and then it was fine.

Hi,

Please ask user to try on another machine if possible.
BTW, there are some peoples got the same issue like yours.
