in jira service management cloud, can a guest user do the approvals - approve and decline?
In my experience, the user must have permission to see issues and the project, so if it isn't a at least a customer, it won't we able to approve de issue

Yes, someone with the ability to see the ticket can be setup to be an approver of the ticket.

can a user with role- customer (not in the user management) => can do the approvals from slack? i can see that from jira view-> we can add that user as approver.

I don't know if they can do them directly from Slack - but thinking probably not.
Yes, you would need to add the user to the Approvers field. You can set it up so that they get an email and can click on the Approve or Decline buttons in the email directly and not have to log in.
