@Suraj Who will be responsible for filling out the intake form? Also, do those customers/users have access to your Jira site and the target space where the work items will be created?
You can try using Jira Forms for your use case and in case the customers don't have Jira access then you might have to ask your admin to enable public access to your Jira site for it to work correctly.

If you are using JSM, Go to your JSM project → Project Settings → Request Types → create a new request type and customize the fields. You get a public-facing portal URL you can share with requesters — no Jira login required for submitters.

we dont have JSM, we need to create a form in Jira, and when the form is filled with details jira ticket needs to be created automatically and later after ticket creation confluence page should be created. this is the requirement could to please help me to complete this request by step by step ?
