how to get list of tings in issue

i have a spare part request screen,

a user need to put more goods name in issue screen, it can be one item, two or more item.

how can i make field and screen to meet above requirement?

You will need an app like the "Table grid editor" to do this, unless just using simple tables in text fields is good enough.
