Can a description field and reporter be added to the screen that sends e-mail notification to the relevant team when a task is created?

Which email? And which project type are you using? Jira Service Management? Jira Software? Jira Business?

Are you Cloud or Data Center? If Cloud, then just create an automation rule to send the notification and add those fields to the email. If you are Data Center, then you can use the Automation for Jira app.

What is the URL of your site? Does it end with atlassian.net? Then you are on Cloud. Does it end with your company domain? Then you are on Server/Data Center?
Are you talking about the creation of a Task in Jira Software? Then we're talking about internal notifications. These are based on the Events within a notification scheme.
Currently, Jira Cloud does not offer a way for you to customize the look and feel of your email notifications. But, there are many third party applications available on the Atlassian Marketplace that can help you.
