It looks like you are running into the following Bug that occurs when adding an attachment to a formatted area of a comment via drag and drop into the new issue view:

The workaround currently is to drop the image outside the formatted area in the comment (i.e. bold or color changed text area) noting:

The comment does not fail to be added if the attachment is added outside a "formatted area"
