 If "3rd Party Vendor Hosted" equals "NO or NA"

Then Vendor Contact Info should not be visible

Hi @sreenivasulu gundagula  - Welcome to the Atlassian Community!

You will not be able to do that with the normal out of the box forms in Business projects. You will need to have Advanced Forms turned on to be able to create a form with conditional logic. 

Go to Project Settings on the project.

If you do not have an option on the left menu for Advanced Forms, then it is probably not turned on for your instance and you will need to request that from Atlassian Support.
