auto-fill jira custom field with data from Confluence

We use Jira and Confluence installed together on a server.
When I create a jira issue related to customer support, I enter the product ID (PRODUCT-XXX). I have a confluence page PRODUCT-XXX in which I have the contact details filled.
What would be the best way to retrieve this data in confluence and automatically fill up the custom field "address" for example, in my jira issue, when I enter the product ID, at issue creation?
