Workflow trigger no longer happening for Bitbucket pull requests

Note: this is Jira and Bitbucket both on the cloud offerings.

I had set up workflow triggers to transition a card when

 - a pull request is opened

 - a pull request is merged

These were working fine a last week, this week they are not working no matter what I try.

I do have a trigger for `branch creation` which I see works fine on the card:

"Automated transition triggered when a branch was created in xxx"

But no matter how much I reference the story in the branch prior, pull request during creation or even the commits nothing is happening. I do see this on the story to be sure the integration is fine:

I have a project that I use to test these integrations, set it up a couple of months ago. Now the transitions didn't work. For some reason all transitions had a "This condition always fail" condition. I removed them and it started working again. Hope this can help.
