Won't allow me to import

Hi all, I'm using a demo version of JIRA.  I created a bunch of custom fields that match my .CVS file. I choose the file then I get the screen where you have to map what you want to carry over.  I click it all and check the box to map the fields but the "next" button stays grayed out.  It won't let me do anything. What am I missing?

Have you made all the mandatory mappings?  Summary etc?

I thought I did but perhaps not.  I really don't understand the concept.  I used JIRA for years but never got into the meat if it before.
