Why is a Single Level Group by Report returning inaccurate resolution data?

When I generate any Single Level Group By Reports the "Progress" bar on the side of the screen is inaccurate and always notes that 0 of X issues have been resolved. Through two search filters I identified recently updated issues associated with one of the projects I manage (screen shot 1). I further used this search to pick out any of the recently updated issues that have been resolved, of which there was one (screen shot 2). However, when using the same search filter to generate a Single Level Group By Report none of the issues are noted as having been resolved (screen shot 3).

I'm not sure what is going on. I've check our transition post-functions and while there are some situations where we've updated our workflows and forgot to appropriately tie some transitions to a change in resolution status (which is why there are some items in the initial search with a status of "Done" though they are not noted as being resolved), in many cases issues are set to be marked as resolved once transitioned to the Done status - further, I feel like the searches I used confirms that at least one of the issues returned from my filter is resolved and yet no issues are marked as having been resolved.

One last note - when generating a Single Level Group By Report with the selected group by status of "Resolution," the report groups all items under the header "Irrelevant" (screen shot 4). I think this is likely a clue to something, I'm just not sure to what.
