Why are JIRA Toolkit Plugin fields not showing?

We are using JIRA OnDemand.  JIRA Toolkit Plugin is enabled in add-on management section:

... but we are not seeing any of the additional fields etc.  For example, if we try this query:

project = "myProject" AND created >= 2015-02-01 AND created < 2015-04-01 ORDER BY "Comments count"

we get an error message: Not able to sort using field 'Comments count'. 

The additional fields are also not showing in the list of 'Custom fields', nor in the Default field configuration list of fields.

What are we missing?

Did you create a custom field under Administration > Custom Fields using the field types from JIRA Toolkit?
