Hi,

I would like to create a form that can be sent via e-mail where users has to fill predefined fields, and map those email form fields into JIRa fields.

If you are using JIRA 4.x , the EzGooey Mail Handler might suit you https://marketplace.atlassian.com/plugins/com.capitalcityconsultants.jira.plugins.ccc-ezgooeymh in term of setting the value of the custom field.
