What is the best way to track missed deadlines?

Looking to understand the best way to track missed deadlines:

• What was the original deadline?
• How many times was it adjusted? 
• What was the final completion date?

Would also be helpful to be able to sort/filter by issue type, sprint, etc. 

Currently, we are considering making a spreadsheet to track these items ... but I'm thinking there must be a way to track within JIRA?
