What are the most common bottlenecks in modern DevOps workflows?

Hi all,
While Devops is designed to reduce development and deployment time, unfortunately there is still always a bottleneck, whether it is testing, approval or deployments.
Sometimes these bottlenecks are introduced gradually as teams try to improve control and quality.

What bottlenecks did you find in your work-flows?
How did you identify and sort them out?

Also, Dev community might have some additional insights about the bottlenecks in general that come with developing apps (which would then apply to general DevOps questions)

But from what I've worked with the clients, it's either:

• How to structure their code?
• How to visualize the code (deployments, incidents related to code...) across platforms? (Jira, JSM, Compass... do a pretty decent job here)
• How to speed up development in general? (that's where AI comes these days)
• What are general test workflows, and how to embed those in Jira?

Again, I'm not that much into DevOps side, but I've heard some clients asking these kinds of things.
