Users keep getting "We were unable to submit your request" from Customer Portal

The issue gets created, but the error occurs and I've been unable to replicate it. It's happening often enough that it's becoming a major problem.

I need help in where to look

In the access logs I can see when user submitted the form post to create the issues.
There were no errors logged at the same time

There was an LDAP sync being performed at the same time, but no errors were logged during the sync.
