Unable to see "Create Issue" on all columns of Kanban Board

In one of my projects, I'm unable to see the "Create Issue" option on all columns of the board.

I had tried to change the workflow of the project and then reset all options the way it was. I still do not see the "Create Issue" option.

I only see the "Create Issue" on the ToDo column. How do I get the "Create Issue" button back on all my columns?

Also instead of a + sign towards the right most corner, I see a gear icon saying configure columns in that project. Can someone help me what I must have done wrong?

Not all projects and boards are created equal. Look closely at the two projects that appear different. Are the both CMP or TMP? Are the both using Scrum or Kanban boards? Feel free to share screenshots here so that I might comment further.
