URGENT - unable to create issue in Jira

I'm trying to submit an issue on Jira, and it's coming up with the error shown in the screenshot. It appears that the boxes for me to fill out more information are not being displayed and so I can't progress with my request. I need to be able to submit this issue asap.

I've tried clearing my browser and data history, and tried on multiple browsers (Chrome, Edge etc) and it's still not working.

It seems that you need to update your screen. Someone has made fields required but nit updated the create screen to provide these fields. Are you an admin? If not you need to reach out to them. If you are go into project settings > screens and add the necessary fields.
