We appear to have some users that don't receive email notifications when they are assigned issues.
Only occurs with some users and not all.
And also I did troubleshoot with Notification Helper but it failed

What do you mean the Notification Helper failed? Did you get an error message?
Can you show us the screen for the Notification Helper so we can see what you filled in and what the results were?
