Screeen Workflow Matching

I need to configure screen and workflow(with status) like sample giving photos. How can I do this?

Adding a screen to a transition is done in the workflow editor. You can add the screen either when you create the transition, or if the transition already exists you can edit it and add the screen. Please have a look at this KB for more information.
