SLA Recipients- Adding a Supervisor to the list

The company I work with wants to use the SLA plugin as an escalation tool.  Meaning when work isn't being completed within a certain time period that 4 people are being notified (The Reporter, Current Assignee, Service Manager, and Reporter's Supervisor).  I have been able to implement 3 of the 4.  The fourth is the Reporter's Supervisor being the issue. 

How do I go about adding the Reporter's Supervisor to the Recipients list on List of Notifications for a scheme?  

Below is a snippet of the Add Recipient window.  Where would I find the database for the User Custom Field Value?  Thank you!image.png

Welcome to Atlassian Community!

Per your description, you have created a custom field value where you add the Reporter's Supervisor on the ticket, however, you're not being able to find it in the Notification scheme of your project, is that correct?

If that's the case, the field should be appearing in the User Custom Field Value as you are displaying in your screenshot. Can you confirm if the Custom field is properly added to your project screens?
