Request FORM in Jira

We want to set up a workflow in JIRA that will allow a specific form to be replicated in JIRA and to automatically create a task based on this.

We will need to customise the fields of the request so it matches a specific form

Any Ideas ?

You can create issue "create" screens that contain the fields you need to capture in your form.
