Why do you ask? If we know, you'll probably get a better answer.
That said, typically, you see something sitting above the Epic level when/if an organization wants to organize its work top-down around something bigger and more abstract than an Epic.
Some organizations even use 2 or 3 levels above the Epic.
These organizations already have a defined process that calls for these higher levels in both cases.
If you don't, there's no reason to dream up a use case for Initiatives (IMO).
